Name maze request form
Use this form to request first names (forenames) be made into a maze. I will email you when they are complete and added to the website. I typically decline any requests for last names, full names, companies, club names or similar. Timing to complete varies based on my schedule and when I see the email, but the turnaround time can be relatively quick, with large requests taking longer.
All mazes are currently in the same format as shown in the name maze section of the site. I format them into pdf downloads, sized to a regular piece of paper. Short names will be larger than longer names as a result.
I make free printable maze books for both kids and adults and have books for sale on Amazon. You can sign up for alerts for when I launch new ones in the form.
